The Benefits of Orthokeratology

2024-08-20

As an optometrist with years of experience, I’ve seen firsthand how dramatically vision correction options have evolved. While glasses and contact lenses remain incredibly popular – and effective – there’s one treatment that consistently surprises people with its potential: Orthokeratology, or Ortho-K.

What is Orthokeratology?

Ortho-K uses specially designed, rigid gas-permeable contact lenses that you wear only while you sleep. It’s not a lens you wear during the day. The lenses gently and temporarily reshape your cornea – the clear front surface of your eye – while you sleep. This reshaping corrects your vision, allowing you to see clearly during the day without glasses or contact lenses.

Think of it like a gentle, reversible mould for your cornea. It's different than LASIK or other permanent refractive surgeries because the effects aren't permanent. If you stop wearing the lenses, your cornea gradually returns to its original shape, and your vision returns to what it was before.

Is Ortho-K Right for Me?

Not everyone is a candidate. Ortho-K is most commonly used to correct myopia (nearsightedness), but it can also sometimes address mild astigmatism. It's generally best suited for:

  • Individuals aged 8 and up. There’s increasing evidence that starting Ortho-K in childhood can actually slow the progression of myopia – more on that later.
  • People with low to moderate myopia (typically up to -6.00 diopters). Higher prescriptions can be more challenging to correct and may not achieve the desired results.
  • Those who are motivated and able to follow a strict lens care and wearing schedule. Compliance is key to success.
  • Individuals who aren’t good candidates for, or prefer to avoid, LASIK or other refractive surgery.

We often see patients in Okotoks and the surrounding areas, including Calgary, who are athletes or those working in professions where daytime contact lens wear is impractical or uncomfortable – construction workers, performers, etc. But it's gaining traction with all sorts of people.

How Does the Ortho-K Process Work?

It's not a one-size-fits-all procedure. Here's what you can expect:

  1. Comprehensive Eye Exam: This is crucial. We need to thoroughly assess your eye health, corneal shape (using a technology called topography), refractive error, and overall suitability for Ortho-K.
  2. Lens Fitting: If you’re a good candidate, we’ll take precise measurements of your cornea to design custom lenses specifically for your eyes. This isn’t something you can buy “off the shelf.” Each lens is unique.
  3. Initial Lens Wear: You’ll start with a pair of ‘fitting’ lenses. You’ll wear these overnight for a short period – often just a few hours – and then we’ll check the fit and initial reshaping.
  4. Gradual Progression: Over several weeks, we'll gradually increase the wearing time as your cornea reshapes. This is a carefully monitored process. Typically, you’ll progress from a few hours a night to wearing the lenses every night.
  5. Ongoing Monitoring: Regular follow-up appointments (usually every 3-6 months) are essential to ensure the lenses continue to fit properly, your cornea is healthy, and your vision remains clear.

The initial fitting process usually takes about 2-4 weeks to stabilize. It's not instant vision correction.

What Are the Benefits Beyond Just Not Needing Glasses?

Many people think of Ortho-K purely as a convenience – waking up and seeing clearly. But there are other advantages:

  • Myopia Control: This is a huge benefit, especially for children. Research suggests that Ortho-K can significantly slow the progression of nearsightedness. This is because reshaping the cornea changes how light focuses on the retina, reducing the stimulus for the eye to continue growing longer (which is what causes myopia to worsen). We're seeing this become a primary driver for parents choosing this option.
  • No Daytime Contact Lens Discomfort: For those who find traditional contacts irritating, Ortho-K eliminates that issue entirely.
  • Reduced Risk of Dry Eye: Because you’re not wearing contacts during the day, there's less irritation and a lower risk of developing dry eye symptoms.
  • Active Lifestyle: Great for swimming, sports, and anyone who wants clear vision without glasses or contacts interfering.

What Does Ortho-K Cost?

Cost is a valid concern. Ortho-K is generally more expensive upfront than traditional glasses or contact lenses. Here's a breakdown (prices are approximate and as of 2026):

  • Initial Fitting & Evaluation: $600 - $1000 (this covers the comprehensive exam, topography, initial lens design, and follow-up appointments during the fitting process). Your insurance may cover a portion of this.
  • Lens Set (Pair): $1500 - $2500 (this is for the custom-designed lenses themselves – you’ll need a new set every 1-2 years, as the design may need minor adjustments).
  • Annual Follow-Up Exams: $150 - $300 per year (to monitor your progress and ensure continued success).
  • Lens Care Supplies: $50 - $100 per year (cleaning solution, storage case).

Compared to the ongoing cost of disposable contact lenses or frequent eyeglass replacements, the long-term cost can be comparable – or even less – for some individuals. It really depends on your individual needs and lifestyle.

What Are the Risks and Side Effects?

Like any medical treatment, Ortho-K isn’t without potential risks. While generally safe, here are some things to be aware of:

  • Infection: As with any contact lens wear, there's a risk of corneal infection. Strict lens care and hygiene are paramount. We provide detailed instructions and emphasize the importance of following them.
  • Corneal Abrasion: The lens can sometimes cause a small scratch on the cornea, particularly during the initial fitting period. This is usually temporary and resolves on its own.
  • Glare and Halos: Some people experience temporary glare or halos around lights, especially at night. This usually diminishes as the cornea adapts.
  • Reduced Best Corrected Visual Acuity: In rare cases, Ortho-K can slightly reduce your overall visual acuity, even with glasses or contacts.
  • Lens Discomfort: Some individuals may find the lenses uncomfortable, even after the fitting period.

We thoroughly discuss all potential risks and benefits with each patient before they begin treatment.

What Most People Don't Realize About Ortho-K

A big misconception is that Ortho-K is a quick fix. It's not. It requires commitment, diligent lens care, and regular follow-up appointments. Also, it’s not a substitute for a proper eye exam. You still need to have your eyes checked annually to monitor your overall eye health.

Another thing: the corneal reshaping is temporary. If you stop wearing the lenses, your vision will gradually revert to its original state. This isn’t a flaw, it’s the nature of the treatment. It provides a reversible solution for vision correction.

Finally, don't assume Ortho-K is a last resort. For the right candidate, it can be a fantastic first step towards achieving clear, comfortable vision.
